Coaching Service Agreement
Thank-you so much for your interest in Breathing Space Home and Life Organizing Solutions for Active Families.
This service agreement outlines the terms and conditions for organizing services, and must be e-signed before services commence.
1. Privacy and Confidentiality
Everything shared in this coaching relationship will be kept confidential unless you are notified and have given consent about sharing beforehand.
I will use you as a reference only with your written permission. I will quote you and/or use your name in promotional literature only with written permission.
2. Billing Rate
Coaching sessions are sold in a package of 4 x 1 hour sessions only for a total of $260 per package.
3. Billing Methods and Terms of Payment
Clients will be invoiced prior to the first coaching session.
100% of coaching fees are due before the first coaching session.
I accept cash, e-transfer or credit card.
Please make all appointments/calls on time. If you must be late, please call me or email ahead of time. If you must change an appointment, please give me 24 hours’ notice and I will reschedule the appointment. If less than a 24-hour notice is given, a makeup appointment will be scheduled if feasible with my week’s schedule. If you miss a scheduled appointment without giving me 24-hour notice, I will need to bill you for it.
5. Your Coaching Sessions
As your coach, it is helpful for me to be aware of past and present events, addictions, conditions, medications, and/or ways of thinking that may influence your life and your ability to undertake your coaching program. (e.g. Are you in a 12-step program? Are you in therapy? Do you have a learning disability? Is there a crisis or loss that happened recently in your life?). To the extent that you are able and comfortable, please share life events that may impact our coaching relationship.
Breathing Space coaching work is a shared responsibility. I hold and expand your options and possibilities while you do the work. Please ask for what you want and let me know what is working and not working in our coaching relationship.
You understand that this is coaching and not therapy. If we arrive at issues that are more suitable for a therapeutic situation, I will suggest that you see a more appropriate professional.
